Prima Registration
Register Before Your First Visit Here
For your convenience we are giving you the option to fill out the registration form before you come in. If you are a new patient or any of your information has changed since your last visit please view the Advance Directive and Privacy Practice information and then print out, fill out, and bring in the Registration Form when you come into our office. We have electronic medical records (Digichart) and require that you fill out your medical record information prior to your appointment. When you make your appointment you will be sent a letter with instructions to get online into the medical records. You will be given the website and a personal code. If you do not have access to a computer, you may come in 45 minutes prior to your appointment and use the patient portal computer in our waiting room. The physicians cannot see you until this is completed.
