How to make an appointment
New Patients
The pulmonary specialists require a referral from your primary care or treating physician before an appointment can be made. Some insurance companies require a pre-authorized referral prior to seeing a specialist. Please check with your insurance plan; heir phone number should be on the back of your insurance card.
It is important that your medical records are available to the pulmonary specialists. You may be asked to bring CDs of CT scans, x-rays, or other digital images. We will be glad to help you obtain these records.
